AutoCAD 2013 - Print out will show disconnections in various layers

Hi guys,

New to the forum and have my first issue.  When printing a drawing out it will show some disconnections on various layers.  I have attached a scan of the image after printing that will make a bit more sense.  The missing items are circled in green.

When the same drawing is sent directly to a PDF, same problem persists (also attached).

I have re-installed the printer drivers but it has not resolved anything.

Any other advice is appreciated.
