Constrain annotative block insert

I am trying to annotate a a dynamic block that can rotate. I would like to use a multileader and constrain the arrowhead to one of the lines that rotate and have the entire multileader object rotate with it. What happens now is the arro rotates and stretches but the text and landing do not move.

As a work around, I created a leader from a couple of lines, an arrow, and some text. This gives me the desired actions but the arrow and lines are not annotative (they do not scale when the viewport changes scale).

I tried creating an annotative block of a leader which does change with the annotative scale but I cannot constrain this to the objects that rotate.

Is it possible to apply geometric constraints to annotative blocks?
